(a) An applicant for a perfusionist license shall submit a sworn application to the Department of Health for consideration. (b) To qualify for the licensing examination, the applicant shall have successfully completed a perfusion education program approved by the State Board of Health. (c) To qualify for a license, an applicant shall pass a competency examination. The examination shall be approved by the board and shall be administered to qualified applicants at least one (1) time per calendar year. (d) No later than two (2) months after the date on which a licensing examination is administered, the board shall notify each examinee of the results of the examination. (e) An applicant who has failed the licensing examination may request in writing that the board furnish the applicant with an analysis of the applicant's performance on the examination.
