(a) Any person who meets the licensing qualifications under this chapter is entitled to receive a license as a licensed perfusionist. (b) The licensed perfusionist shall: (1) Display the license in an appropriate and public manner or maintain an accurate copy of the perfusionist's license on file in the healthcare facility in which the licensed perfusionist is working; and (2) Keep the Department of Health informed of any change of address. (c) A license certificate issued by the department is the property of the department and shall be surrendered upon demand. Acts 1999, No. 888, § 9.
