Permits for mail-order pharmacy

(a) A mail-order pharmacy which dispenses drugs shall register with the board.
(b) A mail-order pharmacy shall submit an application for a permit to the board. The
application shall require the following information:
(1) The owner of the mail-order pharmacy, whether an individual, a partnership, or a
corporation.
(2) The names and titles of all individual owners, partners or corporate officers.
(3) The pharmacy manager.
(4) The pharmacist-in-charge.
(5) The complete address, telephone number and fax number of the mail-order pharmacy.
(c) This section does not apply to any mail-order pharmacy which operates solely as a
wholesale distributor.
