(1) Each applicant for licensure as a hearing instrument specialist shall: (a) submit an application to the division on a form the division approves; (b) pay a fee to the division as determined under Section 63J-1-504; (c) pass an appropriate entry-level examination the division approves by rule in accordance with Title 63G, Chapter 3, Utah Administrative Rulemaking Act; (d) complete at least six months of training with a supervising hearing instrument specialist or supervising audiologist; (e) pass the Utah Hearing Instrument Specialists Law and Rule Examination; and (f) if the applicant holds a hearing instrument intern license, surrender the hearing instrument intern license at the time of licensure as a hearing instrument specialist. (2) Each applicant for licensure as a hearing instrument intern shall: (a) submit an application to the division on a form the division approves; (b) pay a fee to the division as determined under Section 63J-1-504; (c) pass the Utah Law and Rules Examination for Hearing Instrument Specialists; and (d) present evidence acceptable to the division that the applicant, when licensed, will practice as a hearing instrument intern under the supervision of a supervising hearing instrument specialist or supervising audiologist in accordance with Section 58-46a-302.5.
