The board may issue a provisional license to a person who: (1) Except for passing an examination under this chapter, otherwise qualifies for a hearing aid dispensing license; (2) Submits an application on the form prescribed by the board; and (3) Pays the application fee set by the board, pursuant to rules promulgated pursuant to chapter 1-26 , not to exceed one hundred fifty dollars. A provisional license is valid until the board receives the results of the qualifying examination. The board may not renew a provisional license except for good cause shown to the satisfaction of the board. A person who holds a provisional license pursuant to this section may provide hearing aid services only under the supervision of a licensed hearing aid dispenser or licensed audiologist.
