In the presence of ___________________________ ) ___________________________ ___________________________ ) ___________________________ Witnesses Every marriage license must contain the full name of each party before the marriage. Every certificate of marriage must contain the full name of each party before and after the marriage and be signed by two witnesses to the marriage in addition to the signature of the person who solemnized the marriage.
