§ 202. Certificate to be issued to exempt volunteer firefighter. Any\nperson described in sections two hundred or two hundred one of this\narticle shall be entitled to a certificate from the company in which he\nor she served or of which he or she was a member at the time of its\ndisbandment which shall show the date of the entrance of such\nfirefighter upon his or her service, the period of his or her service;\nif he or she was honorably discharged from such company the certificate\nshall so state; that he or she was in good standing in his or her\ncompany at the time of applying for such certificate or at the time he\nor she was discharged therefrom; or that he or she was in good standing\nin his or her company at the time of the disbandment thereof. Such\ncertificate shall be signed by the president, captain, foreman or\nsecretary of the company and shall be acknowledged by such officers\nbefore an officer commissioned to take acknowledgments, and shall also\nhave attached thereto a certificate attested by the affidavit of the\nsecretary, clerk or other person having the custody of the company's\nrecord of membership, that the statements of fact contained in said\ncertificate are true, and the affidavit and acknowledgment shall be\nsubstantially in the following form:\nState of New York )\n ) ss.\nCounty of ............ )\n On this ............ day of ........... , in the year ........... ,\nbefore me personally came ......................... , to me known and by\nme known to be the same persons described in and who executed the\nforegoing certificate and they severally duly acknowledged to me that\nthey executed the same and the said .............................. being\nby me duly sworn, deposes and says, that he/she is ................. of\nthe company aforesaid and is the custodian of the records of its\nmembership and that the facts above stated relating to the service of\nthe person described in such certificate are true.\n ...................................\n Notary Public\n Such certificate so attested shall in all courts of the state and in\nthe offices of all persons clothed with power of appointment or removal\nin the service of this state and in the several cities, counties, towns\nand villages thereof, be presumptive evidence of the facts therein\nstated.\n
