Collaborative practice of pharmacy means the performance of tests to address chronic diseases and public health issues, including, without limitation, outbreaks and occurrences of specific diseases and disorders, by one or more pharmacists in collaboration with one or more practitioners in accordance with a collaborative practice agreement.
