Proof of applicants qualifications

Except
as otherwise provided in NRS 636.206 and 636.207 , an applicant must file with the
Executive Director satisfactory proof that the applicant:
1. Is at least 21 years of age;
2. Has graduated from a school of
optometry accredited or approved by the Board pursuant to NRS 636.135 ;
3. Has passed each part of the
comprehensive national optometry examination administered by the National Board
of Examiners in Optometry or its successor;
4. Has passed each examination identified,
administered or approved by the Nevada State Board of Optometry pursuant to NRS 636.150 ; and
5. Has not been disciplined for harming a
patient as a licensed optometrist in another state.
