1. Upon furnishing information that his or her identification card is lost or destroyed and paying the prescribed fee, the person to whom the original was issued may obtain a duplicate. 2. If the original of a duplicated card is subsequently recovered or a lost card is found, the person having possession shall return it immediately to the Department.
