An applicant for licensure under this section shall pay a fee established by the board by the promulgation of an administrative regulation and shall show to the board that he: (1) Is a person of good moral character; (2) Is twenty-one (21) years of age or older; (3) Has a minimum of a high school diploma or a general education development certificate; (4) Is a citizen of the United States or has status as a legal alien; (5) Is free of contagious or infectious disease; and (6) Has completed the apprenticeship requirements of the formal education as provided by KRS 334.090.
