Request for release of foster child's Social Security number; approval

Sec. 4. (a) The department shall approve a request made under this chapter if: (1) the child lived with the individual making the request in the eighteen (18) months preceding the request; and (2) the child lived with the individual making the request for: (A) at least six (6) months in any one (1) calendar year; or (B) more than one-half (1/2) of the days the child lived in the calendar year if the child was born or died in the calendar year.       (b) If the department approves a request, the department shall: (1) provide the child's Social Security number to the individual; and (2) notify the parents of the child that the child's Social Security number has been disclosed under this chapter if parental rights for the child have not been terminated.
