Expiration and renewal of pharmacy permits

Sec. 22. (a) A pharmacy permit shall expire biennially on a date established by the agency under IC 25-1-5-4 .       (b) If a pharmacy permit lapses for not more than three (3) years, it may be reinstated by the board if the holder of the permit meets the requirements established under IC 25-1-8-6 (c).       (c) If a pharmacy permit has been expired for more than three (3) years, the permit may be reinstated by the board if the holder of the permit meets the requirements for reinstatement under IC 25-1-8-6 (d).       (d) No pharmacy may be open for business after the established biennial renewal date until the permit is reinstated.
