Transfer of ownership or location of pharmacies

Sec. 21. (a) A pharmacy permit is not transferable as to location or ownership.       (b) Not later than ten (10) days after the change of ownership of a pharmacy, an application shall be submitted for transfer of ownership accompanied by a signed and dated certificate of sale. The original permit remains valid until a new permit is issued or the application is rejected by the board. Not later than ten (10) days after notice of the board's action, the old permit is void and must be returned immediately by the new owner.       (c) If the holder of a pharmacy permit desires to change the location of the pharmacy, he shall file an application on a form provided by the board for a permit for the new location.       (d) All applications for transfers of ownership or location of a pharmacy must be accompanied by the appropriate fee.
