(a) Subject to the limitations set forth in this act and upon payment of one hundred dollars ($100.00), any person who adopts a trade name for use in this state may file an application for registration of the trade name in duplicate in the office of the secretary of state on forms furnished by the secretary of state setting forth, but not limited to, the following information: (i) The name and business address of the applicant for registration, and if a corporation, the state of incorporation; (ii) The trade name sought to be registered; (iii) The general nature of the business in fact conducted by the applicant; (iv) The signature of the applicant acknowledged before a notarial officer. (b) Upon compliance by the applicant with the requirements of this act, the secretary of state shall return a duplicate copy of the application for registration to the applicant stamped with the date of filing.
