(1) Every application, in order to entitle the applicant to appear for examination, or to be examined, must state the facts under oath on the following subjects: (a) Full name, residence address and post-office address. (b) Citizenship. (c) Age. (d) Place of birth. (e) Health and physical capacity for the public service. (f) Previous employment in the public service. (g) Business or employment and residence for the previous 5 years. (h) Education. (2) In addition to the information required under sub. (1), the applicant shall furnish any other information that may reasonably be required by the board concerning the applicant’s fitness for the public service.
