Wisconsin Code § 180.1006

Articles of amendment

A corporation amending its articles of incorporation shall deliver to the department for
filing articles of amendment that include all of the following
information:
(1) The name of the corporation.
(2) The text of each amendment adopted.
(3) If an amendment provides for an exchange, reclassification or cancellation of issued shares, provisions for implementing
the amendment if not contained in the amendment itself.
(4) The date on which each amendment was adopted.
(5) A statement that the amendment was adopted in accordance with s. 180.1002, 180.1003 or 180.1005, whichever is the
case.
