Display of license, permit or certificate

(a) The board shall prescribe the form for a license, permit and certificate and may issue a
duplicate upon payment of a fee.
(b) Any person, not employed by the bureau or a municipal or county health department,
shall conspicuously display his or her license, permit or certificate at his or her principal
place of practice.
(c) A person regulated by the board shall carry valid proof of licensure, permit or certificate
on his or her person during the performance of his or her duties.u
