Whenever the Commission revokes or suspends a license issued pursuant to this chapter, it shall: 1. Enter an order to that effect, which order shall set forth its findings; 2. Compile a record containing (i) a summary of the evidence, (ii) the findings with respect thereto, (iii) the order, and (iv) the reasons supporting the revocation or suspension; and 3.
