An applicant for licensure as an acupuncturist shall: (1) submit an application to the division on a form the division approves; (2) pay a fee to the division that the division determines in accordance with Section 63J-1-504; (3) meet the requirements for current active certification in acupuncture under guidelines the National Certification Board for Acupuncture and Herbal Medicine (NCBAHM) establishes as demonstrated through a current certificate or other appropriate documentation; (4) pass the examination the division requires by rule the division makes in accordance with Title 63G, Chapter 3, Utah Administrative Rulemaking Act; (5) establish procedures, as the division defines by rule the division makes in accordance with Title 63G, Chapter 3, Utah Administrative Rulemaking Act, which shall enable patients to give informed consent to treatment; and (6) meet with the board, if requested, for the purpose of evaluating the applicant's qualifications for licensure.
