Utah Code § 58-41-401

Grounds for denial of license -- Resumption of practice

(1) The division may:
(a) refuse to issue a license to an applicant;

(b) refuse to renew a license;
(c) revoke, suspend, or restrict a license;
(d) place a licensed individual on probation;
(e) issue a public or private reprimand to a licensed individual; or
(f) issue a cease and desist order.
(2) A licensed individual who has a license revoked may submit an application for licensure to the
division after one year from the day after the day on which the division revoked the licensed
individual's license.
(3) The division shall perform an act listed in Subsection (1) in accordance with Section 58-1-401.
