(1) To register under this part, a person shall:
(a) pay an original registration fee established by the department in accordance with Section
63J-1-504; and
(b) submit a registration statement, in a form approved by the department, that contains the
information described in Subsection (2).
(2) A provider's registration statement shall include:
(a) the provider disclosure described in Section 31A-44-301;
(b) a copy of the continuing care contract that the provider will propose to a prospective facility
resident;
(c) evidence that the provider's facility is located or will be located in a zone that a municipality or
county has zoned for continuing care facilities; and
(d) information required by the department by rule made in accordance with Title 63G, Chapter 3,
Utah Administrative Rulemaking Act.
(3) The department shall accept or deny a registration no later than 180 days after the day on
which the provider applies for registration.
