Utah Code § 26B-4-516

Standing prescription drug order issued by the department

(1) The department may only issue a standing prescription drug order if the prescription:
(a) is limited to a clearly defined clinical indication;
(b) is for a diagnosis for which the medication has been approved by the federal Food and Drug
Administration; and
(c) is clinically appropriate.
