Contents of resolution or petition

(1) The resolution of the governing body or the petition of the electors shall include:
(a) a statement fully describing each of the areas to be included within the consolidated
municipality;
(b) the name of the proposed consolidated municipality; and
(c) the names of the municipalities to be consolidated.
(2)
(a) The resolution or petition shall state the population of each of the municipalities within the
area of the proposed consolidated municipality and the total population of the proposed
consolidated municipality.
(b) The population for each municipality under Subsection (2)(a) shall be derived from:
(i) the estimate of the Utah Population Committee created in Section 63C-20-103; or
(ii) if the Utah Population Committee estimate is not available, the most recent official census
or census estimate of the United States Bureau of the Census.
(3) The first page of a petition described in this section shall include the following statement in at
least the same size type as the majority of the other statements on the page:
"WARNING TO SIGNERS WITH PRIVATE VOTER REGISTRATION RECORDS

If you sign this petition, your voter identification number and the date you signed may be
publicly disclosed. This disclosure may occur even if you are an at-risk voter with a voter
registration record that has been classified as a private record."
