Sec. 844.301. APPLICATION FOR DISABILITY RETIREMENT. (a) A member may apply for disability retirement by filing a valid application for retirement with the retirement system. (b) An applicant must provide medical and other pertinent information for evaluation by the medical board and submit to medical examination as required by the medical board.
