Sec. 802.102. APPLICATION. An applicant for a license under this chapter must: (1) submit to the department a completed application on a form prescribed by the department; (2) submit to the department the information regarding the applicant's facilities and operations requested by the department; (3) demonstrate that the applicant has satisfied the requirements of this chapter and rules adopted under this chapter; and (4) pay to the department the required fee.
