Sec. 681.0031. APPLICANT'S IDENTIFICATION. (a) The applicant shall include on the application the applicant's: (1) driver's license number or the number of a personal identification card issued to the applicant under Chapter 521 ; (2) military identification number; or (3) driver's license number of a driver's license issued by another state or country if the applicant is not a resident of this state and is seeking medical treatment in this state. (b) The county assessor-collector shall record on any disabled parking placard issued to the applicant the following information in the following order: (1) the county number assigned by the comptroller to the county issuing the placard; (2) the first four digits of the applicant's driver's license number, personal identification card number, or military identification number; and (3) the applicant's initials.
