Sec. 623.080. CONTENTS OF PERMIT. (a) Except as provided by Subsection (b), a permit under this subchapter must include: (1) the name of the applicant; (2) the date of issuance; (3) the signature of the director of the department; (4) a statement of the kind of equipment to be transported over the highway, the weight and dimensions of the equipment, and the kind and weight of each commodity to be transported; and (5) a statement of any condition on which the permit is issued. (b) A permit issued under Section 623.071 (a) or (b) must also state: (1) each highway over which the equipment is to be transported, if the permit is for a single trip; or (2) the region or area, as required by rule, over which the equipment is to be operated, if the permit is for other than a single trip.
