Sec. 411.526. NOTICE OF VIOLATION AND PENALTY. If, after investigation of a possible violation and the facts surrounding the possible violation, the department determines that a violation occurred, the department shall issue to the respondent a notice of alleged violation stating: (1) a brief summary of the alleged violation; (2) the amount of the recommended administrative penalty; and (3) that the respondent has the right to a hearing to contest the alleged violation, the amount of the penalty, or both.
