Sec. 403.078. FORM. All claims and accounts against the state shall be submitted on forms or according to the method and format that the comptroller prescribes. The claims and accounts shall be prepared to provide for entering on the claim or account, for use of the comptroller's office, the following: (1) authorization of the head of the office or other person responsible for the expenditure; (2) the appropriation against which the disbursement is to be charged; (3) information required by the comptroller's rules; (4) proof that the claim or account was presented to the state within the period of limitation provided by Section 16.051 , Civil Practice and Remedies Code, or other applicable statute; and (5) other appropriate matters.
