Sec. 401.503. APPLICATION PROCESS. (a) An application for a certificate or license under this subchapter must be submitted in the manner and on a form prescribed by the executive director. (b) The application must require an applicant to provide sworn statements relating to the applicant's education and to provide other information required by the commission.
