Sec. 206.214. LICENSE HOLDER INFORMATION. (a) Each license holder shall file with the medical board: (1) the license holder's mailing address; (2) the address of the license holder's residence; (3) the mailing address of each of the license holder's offices; and (4) the address for the location of each of the license holder's offices if that address is different from the office's mailing address. (b) A license holder shall: (1) notify the medical board of a change of the license holder's residence or business address; and (2) provide the medical board with the license holder's new address not later than the 30th day after the date the address change occurs.
