Sec. 1152.154. REGISTRATION APPLICATION; FEES. (a) An applicant for registration must file an application with the department on a printed form prescribed by the executive director. (b) The application must be accompanied by: (1) a nonrefundable application fee; and (2) a registration fee. (c) The department shall refund the registration fee if the executive director does not approve the application.
