Sec. 1051.456. NOTICE OF BOARD ORDER. The executive director shall give notice of the board's order to the person on whom the penalty is imposed. The notice must include: (1) the findings of fact and conclusions of law, separately stated; (2) the amount of the administrative penalty ordered, if any; (3) a statement of the right of the person on whom the penalty is imposed to judicial review of the board's order; and (4) other information required by law.
