Sec. 1002.253. LICENSE APPLICATION. (a) An applicant for a license under this chapter, including an applicant for a temporary or reciprocal license, must apply on a form prescribed by the board that is signed by the applicant. (b) The application must include: (1) information concerning the applicant's education; (2) a detailed summary of the applicant's relevant work experience; and (3) a signed statement that the applicant has read and will comply with the code of professional conduct adopted under this chapter. (c) The application must be accompanied by the appropriate application fee.
