(a) Application for licensure shall be made to the board in writing or via online application. (b) The application shall be accompanied by the applicant's photograph and shall state the name, age, sex and place of residence of the applicant, the name and location of the school or college from which the applicant was graduated, the length of time devoted to the study of chiropractics and the date of graduation. (c) All applications shall be signed and sworn to by the applicant. (d) (1) When the board receives a completed application for initial licensure from an applicant, then the board shall, within sixty (60) days from the date the board receives the completed application: (A) Render a decision on the application; or (B) Inform the applicant of the need to appear before the board.
