Notice of lien--Contents--Place of filing

44-13-1
shall be effective, however, unless a written notice containing the name and address of the person receiving such ambulance service, the date such service was rendered, and the name of person or persons alleged to be responsible for payment for such services shall be filed in the office of the register of deeds of the county in which the person receiving such services resides, and in the county of his legal residence or last residence if he is deceased.
