Expired license--Reactivation

An expired license may be reactivated within the four-year period following its expiration if the applicant:

(1)

Pays any applicable renewal fee required for the period of expiration;

(2)

Provides proof of any continuing education required for the period of expiration; and

(3)

Provides proof of passing a national examination approved by the board after the date the license expired.
