In an application for a certificate D the applicant shall state, besides the other matters by law provided: (1) The carrying capacity as given by the manufacturer of the motor vehicle; (2) The route on which the motor vehicle is to be used; (3) Whether reserve or substitute cars are maintained by the applicant to be used only in emergencies and, if so, the number of such reserve and substitute cars and a complete description of each; (4) The length of the route in miles on improved public highways in this State; (5) The weight of the vehicle when empty; and (6) The schedule under which the vehicle shall operate during the ensuing year.
