All complaints concerning a licensee’s business or professional practice shall be received by either the licensing board or the department of health. Each complaint received shall be logged, recording at a minimum the following information: (1) Licensee’s name; (2) Name of the complaining party; (3) Date of complaint; (4) Brief statement of complaint; and (5) Disposition.
