Long-term care assessment and care management services — Program establishment

There is established, within the office of healthy aging, a long-term care assessment and care management program. The purpose of the program is to provide chronically impaired or disabled persons in need of assistance with activities of daily living with an assessment of need and guidance in the procurement of necessary long-term care and supportive services and to make these persons aware of available long-term care service options. The department of human services shall cooperate and assist in the establishment of this program.
