(a) Licensing boards.-- A licensing board shall administer this chapter as to prescribers under its jurisdiction. (b) Bureau.-- (1) The bureau shall, in cooperation with the licensing boards, prescribe the form under section 52A04(a)(3) (relating to procedure). (2) The bureau shall transmit notice of the availability of the form to the Legislative Reference Bureau for publication in the Pennsylvania Bulletin.
