(1) The Health Licensing Office shall issue a certificate to each applicant who qualifies under ORS 690.046 or 690.047. The certificate is prima facie evidence of the right of the holder to practice in a field of practice for which the holder has qualified and purports to be a practitioner. (2) A certificate must show the practitioners name, certificate number, expiration date and field of practice for which the practitioner is qualified to perform services. (3) A practitioners certificate shall indicate any specialty certification, obtained under ORS 690.047, for which the practitioner is qualified to provide advanced services in a field of practice. Advanced services shall be defined by rules adopted by the office.
