A person desiring to obtain a certificate or demonstration permit shall apply in writing to the Health Licensing Office on a form approved by the office. Each application shall contain an affirmation by the applicant that the information contained therein is accurate. The application must also include evidence establishing to the satisfaction of the office that the applicant possesses the necessary qualifications. Note: The amendments to 690.035 by section 4, chapter 117, Oregon Laws 2025, become operative January 1, 2027. See section 6, chapter 117, Oregon Laws 2025. The text that is operative on and after January 1, 2027, is set forth for the users convenience. 690.035. In order to obtain a certificate, provisional certificate or demonstration permit, a person shall apply in writing to the Health Licensing Office on a form approved by the office.
