A physical therapist or physical therapist assistant shall report to the Oregon Board of Physical Therapy a change to the physical therapists or physical therapist assistants name, business or residential address, contact email address or contact telephone number within 30 days after the date of the change. Note: 688.138 becomes operative July 1, 2027. See section 24, chapter 627, Oregon Laws 2025. Note: 688.138 was added to and made a part of 688.010 to 688.201 by legislative action but was not added to any smaller series therein. See Preface to Oregon Revised Statutes for further explanation.
