(1)(a) When entering a judgment in an action for a violation or a misdemeanor, a justice court shall, by including a notification in the judgment document or by another effective manner, notify the defendant that the defendant may request the name and contact information of the government official or entity upon which service of a notice of appeal is required under ORS 55.170, with instruction on how to make such a request. (b) Upon the defendant making a request described in this subsection, the justice court shall provide the defendant with the name and contact information of the government official or entity upon which service of the notice of appeal is required. (2)(a) When entering a judgment in a civil action, a justice court shall, by including a notification in the judgment document or by another effective manner, notify the parties that a party may request the name and contact information of the adverse party upon whom service of a notice of appeal is required under ORS 55.170, with instruction on how to make such a request. (b) Upon a party making a request described in this subsection, the justice court shall provide the party with the name and contact information of the adverse party upon whom service of the notice of appeal is required that is in the possession of the court at the time the request is made.
