revoke or suspend certificate - Grounds - Notice and proceedings. The Department shall have power to refuse to issue, renew, reinstate or reactivate or, after notice and opportunity for an individual proceeding as provided in the Administrative Procedures Act, the Oklahoma Environmental Quality Code and rules of the Board, to revoke or suspend any certificate for good cause including but not limited to: 1. Gross inefficiency or incompetence; 2. Violation of any provisions of the Waterworks and Wastewater Works Operator Certification Act or applicable provisions of the Oklahoma Environmental Quality Code, rules promulgated thereunder or the terms of any certificate or order issued pursuant thereto; or 3. Fraud or misrepresentation in obtaining a certificate.
