Within three (3) days after entry into the custody of the
Department of Corrections, every inmate shall complete a notarized
form provided by the Department of Corrections designating the name
and address of any person who is to receive the personal property of
the inmate held by any state correctional facility if the inmate
dies.  Each inmate shall have the opportunity to change the
designation if he or she desires.  In the event of the death of an
inmate, the Department of Corrections shall deliver all money and
personal property found within any state correctional facility
belonging to the inmate to any person designated pursuant to the
provisions of this section.  In the event the inmate’s designee
declines to accept the inmate’s remains and the Department of
Corrections pays for cremation, the costs of the cremation shall be
deducted from any balance due to the inmate’s designee.
