Oklahoma Code § 45-418

Title 45. Mines And Mining: Loss of life or injury - Duty of operator and inspector

Whenever loss of life or serious personal injury shall occur in
or about any said mines, it shall be the duty of the operator having
charge of such mine to report said loss of life or injury without
delay to the deputy mine inspector in whose district such mine is
situated; and the said inspector shall if he deem it necessary
immediately go to the scene of said accident and render such
assistance as he may deem necessary for the safety of the men; and
the said inspector shall investigate and ascertain the cause of such
loss of life or injury and make a report thereof to the Chief Mine
Inspector of the State of Oklahoma, a copy of which he shall
preserve with the other records of his office.
