Title 36. Insurance: Definitions

As used in the Life, Accident and Health Insurance Policy
Language Simplification Act:
1.  "Policy" or "policy form" means any policy, contract, plan
or agreement of life insurance, health and accident insurance, or
subscriber certificates of medical care corporations, health care
corporations, hospital service associations, or health care
maintenance organizations, delivered or issued for delivery in this
state by any company subject to the Life, Accident and Health
Insurance Policy Language Simplification Act; any certificate,
contract or policy issued by a fraternal benefit society; any
certificate issued pursuant to a group insurance policy delivered or
issued for delivery in this state; and any evidence of coverage
issued by a health maintenance organization.
2.  "Company" or "insurer" means any entity authorized to do the
business of life insurance, accident and health insurance; a
fraternal benefit society; a medical care corporation; a health care
corporation; a hospital service association; or a health maintenance
organization.
